Geriatric Assessment Program

Provides specialized geriatric health assessment and treatment to seniors age 65 and over who are frail, at risk, or exhibit complex and/or multiple problems. Provides both the client and their health care providers an overall picture of their health status, including: physical and medical condition; psychological assessment including memory loss and depression; social well-being including support networks; and detailed review of all medications to check for side-effects or interactions. Program hours are 8:30 am to 4:30 pm Monday to Friday.
