Provides specialized geriatric health assessment and treatment to seniors age 70 and older with complex physical, cognitive, and chronic disease management challenges. Provides both the client and their health care providers an overall picture of their health status, including: physical and medical condition; psychological assessment including memory loss and depression; social well-being including support networks; and detailed review of all medications to check for side-effects or interactions. Referral required from physician or nurse practitioner. Program hours are 8:30 am to 4:30 pm Monday to Friday.
