Provides assessment and treatment for older adults age 65 and over with dementia-related behaviour problems or complex psychiatric and medical needs requiring multidisciplinary care. Services include case management, outreach, and rehabilitation services. Also provides family support and education. Care is coordinated with primary care physicians, elder care facilities, and other community programs. Services are provided on an outreach basis in the community. Accepts referrals from physician or nurse practitioner. Serves Richmond. Hours are 8:30 am to 4:30 pm Monday to Friday.
