The Outreach Team offers case management and outreach support to individuals who are homeless, at risk of homelessness, difficult to house, or experiencing crisis situations. On-site supports include weekly access to a nurse, nurse practitioner, physician, and income assistance worker. The team assesses housing needs, provides referrals appropriate services, and assists with obtaining identification and income assistance. Staff support individuals through housing applications, appointments, viewings, move-in process, and liaising with landlords; rent subsidies may be available to assist eligible participants. Clients with a history of addiction are expected to be at least 90 days sober prior to residency. Offers suites for 27 women and 57 men. Units contain a personal kitchen and washroom; shared laundry facility. Length of stay is 24 months. Residents are assigned a program worker in order to pursue a Personal Development Plan, which helps them to find relevant resources and guides them towards their future goals and aspirations. The plan is complemented by in-house group activities. Cost is the shelter portion of income assistance, or 30 percent of earned income. It includes life skills training, individual and group counselling, peer support, and alumni meetings. This phase allows for school attendance and participation in a volunteer work experience program. Phase 3 is a supportive residential program open to men who have completed Phase 1 and 2 at Harbour Light. Services include housing and meals. Clients continue with counselling to assist in the development of an exit plan. Clients are required to participate in group meetings and outside 12-step meetings.
